Web Based SQL Import Tool

Hello,

I have been looking for a web-based import script/tool that will allow
someone to browse for and select a local source file and on submission to the
"form" parse the source file into a destination database. I've found some
"fat-client" tools, but I need something that can run on a website.

Is anyone aware of something like this? It doesn't have to be a commercial
product. Just finding a basis for a script would be a start.
