How do I create a form in an outlook appointment?

I work as an administration officer and one of my jobs is diary management
for about 6 counsellors & 4 case workers. I manage thier appointments and
gather statistics from the information in them. Is there some way that i can
create an appoinment template for them with something like a form in the body
of it that automatically shows. I would like them to be able to fill this
form out for each appointment. This way they would not have to think about
what to put into each appointment as they make it.  It would make information
gathering much easier and I could get exactly the information I need without
chasing anyone up.

You might try posting this to microsoft.public.vb.ole.automation or if
there's
an outlook programming group or office programming group. You might get
better response there.
