What I need is some of your thoughts on the application architecture.  Here
are the requirements I need to satisfy:

Requirements:

1.       Simple, easy to use and maintain
2.       .Net 2.0
3.       Must run as web page
4.       Must also support running as a stand alone application for some
hospitals that want to use it without sending information to the state
(storing data locally)
5.       Must allow creation, maintenance for specific canned questionnaires
6.       Must allow for custom questions that can be added to existing
questionnaires
7.       Must support Tablets or Laptops being used to collect data in the
field
8.       Must support aggregation of data at a central repository

Possible Tools:

1.       InfoPath
2.       Smart Client technology
3.       Web pages
4.       Web services
5.       BizTalk
6.       XML and XSLT
7.       ??
